We conduct a thorough assessment — always with the individual at the centre. We explore their goals, daily living needs, communication preferences, sensory considerations, existing support networks, and what independence looks like for them specifically.

We create a personalised support plan agreed with the individual and — where appropriate — their family and clinical team. The plan is clear, practical, and written around the person's own words, goals, and preferences.
